Responsibility
Parents or care givers should remain easily accessible while children under the age of 18 years participate in all D’Vine Gymsport classes. D’Vine Gymsports holds no responsibility for children dropped off early or picked up late from class. D’Vine Gymsports accepts no responsibility for children outside the building including carpark or other areas, as the parent and or caregiver must maintain responsibility and safety of the child.
Parking and carpark safety
Please ensure that you are taking careful consideration whilst walking around the car park and when entering, driving through and exiting at all times. Remember that children must learn the correct car park safety but is not the staff’s responsibility and should be encouraged at all times from parents/caregivers. There should be no parked cars in front of any fire exits or the roller door at any given time.
Conduct
D’Vine Gymsports reserve the right to cancel any memberships, ask persons to leave the building if staff deem that this is the most appropriate action needed. If poor behaviour is shown including any acts of violence threats that may harm those within the facility, police will be called.
Purchases
When Purchasing any services or goods, it is agreed that any credit or bank account information is true and complete. Additional Charges that may be incurred by credit card companies or others will be the member, participant or purchaser’s responsibility. By signing this agreement, authorisation is given to D’Vine Gymsports and or associated billing company to charge relevant fees through a bank or financial account provided.
Charges
D’Vine Gymsports reserves the right to increase fees and charges with 30 days notice.
Make-Up Classes- Recreational
Make up classes are given if 6 Hours or more notice is given and is dependant on class availability.

Foundation & Development
Make up classes are given if 6 Hours or more notice is given and is dependant on class availability.
Regional Development Program, National Development Program, XCEL High Performance, Junior and Senior Sports Aerobics
Athletes are scheduled 2 Weeks Holidays throughout the year but if additional and unavoidable time off is needed this can be further discussed with the Gymnastics Manager.
ACKNOWLEDGEMENT OF RISK:
I am the parent and or guardian of the participant names on this document. I recognise that Minor and or severe and catastrophic injuries can include but are not limited to strained muscles, broken bones, permanent paralysis or death in gymnastics/sports activities that involve height and or motion; those activities include but are not limited to gymnastics, tumbling, trampoline, movement education, dance and stunting. I also understand that my child(ren)s will be training on all apparatuses and various training devices. I clarify that I deem my child competent and fit to participate in all gymnastics based activity.
CONSENT AND ASSUMPTION OF RISK
Being fully aware of these dangers, I hereby give my child(ren)s consent to participate in any D’Vine Gymsports program and activity and I ACCEPT ALL RISKS associated with this participation.
WAIVER AND RELEASE
In consideration for my or my child(ren)s participation, I hereby for myself and my child(ren)s and our respective heirs and successors, PROMISE NOT TO SUE AND FOREVER RELEASE AND DISCHARGE D’Vine Gymsports, It’s officers, directors, shareholders, employees, contractors, teachers, coaches and volunteers from all liability resulting from damages or injuries incurred as a result of participation in in D’Vine Gymsports activities/programs, including those resulting from acts of negligence.
CONSENT TO MEDIACAL TREATMENT
In the event of an accident or emergency, I hereby authorise D’Vine Gymsports and its representatives, including its employees, contractors, teachers, coaches, and volunteers, to render first aid to my child(ren)s to the extent they deem appropriate, I further authorise D’Vine Gymsports and its representatives to transport or arrange transport, by ambulance if D’Vine Gymsports deems it appropriate for my child(ren)s to attend a hospital or any other medical facility. I additionally hereby agree to be personally responsible for any payment that may incur, including but not limited to medical expenses and transportation that may be via ambulance services of any kind.
Spectators media permissions
Under no circumstances is there to be any photos or videos taken of any person/persons within the facility from a spectator/parent or guardian unless permission is given by senior management of D’Vine Gymsports.
Staff of D’Vine Gymsports media permissions/ PHOTO AND MEDIA RELEASE
I am aware that individual and group photos and videos are taken throughout programs/activities whilst at D’Vine Gymsports, from D’Vine Gymsports staff and associates that may include professional photographers and or videographers. I hereby grant permission for my or my child(ren)s images to be used in publicity, media usage including but not limited to social media, website, training tools and advertising on behalf of D’Vine Gymsports and or our governing bodies.
Cancellation
Cancellation of direct debit requires a minimum of 30 Days notice on a cancellation form on site or confirmed in an email. Members must be aware they are not cancelled until they have received confirmation that the cancellation request has been approved if there are any outstanding fees members will not be cancelled until all payments are recovered and Direct debits will automatically continue until the cancellation form has been approved.
